Simple question tricky answer.  Depending on how the rating is determined the answer is yes or no.

100% service connected for a heart condition YES.

100% solely for a mental health condition NO.

100% combined service connected YES.

Keep in mind that a veteran rated 100% and continues to work runs the risk of VA re-evaluating his/her ratings.

Petes answer was good.  

Max Cleland is a good example of a Veteran who was 100% disabled, and worked full time as a senator!  If you overcome a physical disability such as blown off legs and still manage to work, then, yes, you could receive 100% and work.  

However, if you are TDIU, 100%, then you will probably forefeit at least a portion of your Veterans benefits if you are working.  

Also, as Pete said, if you are disabled due to a mental condition, such as PTSD, depression, etc., then you need to understand that your benefits could be reduced if you worked full time, as you would no longer meet the critieria of "total occupational impairment" if working.
