I agree with Todd, failure to get all 3 legs of the Caluza triangle is almost an "automatic" denial.  

There are at least 2 other denial reasons that are very very common:

1.  VA loses, or the Veteran fails to submit key evidence.  If the VA does not have "documentation" of your disability, then they will deny it.  

2.  VARO's inability to read.  In order to become a rating specialist, you need to be able to read at a kindergarten level or less.  The VA fires any rating specialist who can read, immediately, as this would cost them a bunch of money.  A perfect rating specialist is one who can rubber stamp "claim denied" on claims without reading them quickly.
