0 Moderator brokensoldier244th Posted April 22, 2020 Moderator Share Posted April 22, 2020 (edited) You need proof of service, a discharge of something other than dishonorable, and proof of some kind that the event in question happened when on active duty, whether it be service medical records, private records from same period. Line of duty statement (if relevant) , and a diagnosis that ties the accident to your back to establish theclaim basics. Something from you doctor now saying there same thing would help a lot too. Then you can file a claim on a 21- 526-ez as a fully developed claim (Or not, there is the option not too also if you are missing some info that will take time too) VA can pull all of that on their own if you give them the dates of service and private medical treatment info (dates, name of provider, location) Look at the form, it's pretty simple. DONT FORGET To SIGN IT) If you have a POA working for you put that info on there. 2142 and 2142a forms for medical release are also needed. If you have dependents there is another form for that.
