He wants to get medical records, tax statements, and who knows what else. My idea is that I will sign only release forms that state who they are being sent to. Any thoughts? Then make copies of those for my records. Yes, I do have an attorney and he should have known that I wouldn't sign any blank releases. I called him and told him that the VA has all my medical records it needs and if they want something else, to send us a release with the name of who it is going to then I'll decide if the info is relevent to my case, but there is no way in #*$% that I am signing anything blank. My attorney said they will need my tax returns because I have claimed lost wages. I don't care about the tax thing, but the medical records are private and I was told that the US attorney can request that the judge compell me to sign the releases, but I don't have to and I won't. I was at the Indianapolis, Indiana VA Regional Office, filling out a form 95. The VAso told me to leave block 12d. blank. Both in that block, and the instructions on the reverse side, it explains very clearly, that not entering an amount will make your claim invalid, by leaving it blank, you may loose your rights ! http://www.usdoj.gov/civil/forms/SF95.pdf Geese he was very insistant, ending it with, "WELL if I were you I would leave it blank." Geeze some of those that we have to deal with ought to be in prison...otta be.... God can you imagine the advise he would give a veteran suffering tramatic brain injury... The VA did award me 20% disability in my shoulder(claimed tort) for my Non-Service Connected Pension. I'm using this ammo,(they're own bullets) in addition to other evidence I submitted.
