Question

I saw my prim. care doc Thurs. and told him about my recent bypass. He was nice and polite, ask me the date I had it and if I was placed on any new medicine. That was it. But before my visit with him I had to fill out a form asking me if all was correct. The form showed how many years of active service I had, the different percentages of service connected disabilities, my address, my wives address and some other questions. Have any of you had to fill out this form?

It sounds like just an update of the information he/she has. I frequently have to do that with doctors I use in the private sector (paid by Tricare). They ask much of the same except for the percent disability, etc.
