You can claim it on a standard 21-4138 form. Just be sure to be crystal clear about what tyou are asking for. 121212 Posted July 16, 2009 Share Posted July 16, 2009 Have to read the regulation on Surgical/convalescent pay, it has a lot of stipulations in it, like it has to be a major joint or in a cast. you should read it before you send it in and make sure you get everything on the form, they are picky about it. I had ankle surgery on the 6 of April and I sent all the paper work and the doc did to and they have not mad a decision on my clam. I hope every thing goes right on you claim.
