Paragraph 30: Surgical/convalescent

Question

Had SC knee scoped last week and will be missing 4-6 weeks from work convalesing. Does anyone know what info from the surgen is required to file under Sec 30? Any advise would be appreciated. Thanks!

Get a copy of the OR rerport and the discharge instructions from the hospital.

You can claim it on a standard 21-4138 form. Just be sure to be crystal clear about what tyou are asking for.

Have to read the regulation on Surgical/convalescent pay, it has a lot of stipulations in it, like it has to be a major joint or in a cast. you should read it before you send it in and make sure you get everything on the form, they are picky about it. I had ankle surgery on the 6 of April and I sent all the paper work and the doc did to and they have not mad a decision on my clam. I hope every thing goes right on you claim.

Get your doctor to write a definitive time period giving dates. Otherwise they will develop for it. Example: July 3 - August 15 2009.
