Of course, VA claims to have fixed the problem.  (Hmm, so VA waits 2 years to fix it, and I wonder how many Vets were operated on in those 2 years).  But, now that the media gets a hold of it, then they decide to fix it.  

Next, VA media will say this is an isolated incident, at a single VAMC, that it was caused by poorly trained junior employees, and that the need more money to fix it.   Next year, it will be the same old, same old, with the same management, probably transferred to YOUR VAMC.   But it will be a while until VAOIG finds it, because, well, the VAOIG takes care of its own employees.
