  1. Buck, It wasn't the VA form DBQ. It was actually the insurance long term disability form. I don't believe a MH or PTSD Psychiatrist can fill out the DBQ at request. It would have to be done by assignment to a person that has been placed in the position to complete the DBQ's. This prevents a DBQ from being filled out with prejudice. VA Directive 2008-071 established a policy requiring VHA health care providers, when requested, to assist veteran patients in completing non-Department of Veterans Affairs medical forms by providing the veteran patient with medical statements with respect to their medical conditions and functionality. This includes medical forms from insurance and SSI. Although the VA Directive 2008-071 expired October 31, 2013, it is still required under CFR 17.38(a)(1)(xiv) medical benefits package. This Directive paved the way by establishing the policy and now it is at the discretion of the VA Medical Center to establish and maintain a policy that still meets the requirements of CFR 17.38(a)(1)(xiv) medical benefits package. I did not know this information before I posted in the forum but want to share it with all in the event they run into or have ran into the problems I was having.

Followed the advice and went to Patient Advocate and found my request for the doctors to fill out the paperwork for LTD had already be channeled her way and the doctors were approved to fill them out. I was told to go see my doctors so I did. My psychiatrist explained his reluctance was due to VA policies and he had to have approval first. After sitting down with him and receiving his assessment I feel a lot better. Patients conditions: PTSD, Major Depressive Disorder, Panic Disorder and Psychotic Disorder. (Man I must be F"d" in the head. Description , how does it does it affect interpersonal relations and job? "Patient with very sever anxiety secondary to PTSD with episodes of panic, depression and psychotic disorder. Patients symptoms result in incapacity to maintain healthy interpersonal relationship at work. Severity of symptoms increase when he is away from his home. Patient is not capable of functioning in his occupation due to symptom severity. Patients symptoms appear to be chronic. At this time have been treatment resistant." I guess this will be good. He also checked the box of severe limitation, incapable of minimal activity or sedentary work. Checked the box House confined as well. Thank you for the advice.
  3. Thanks guys, The condition is my PTSD. I have been treated for it for years with medications and counseling but it has been unsuccessful. I go to weekly treatment and my psychiatrist has noted the condition as severe. Next month I will check into the RRTP the live in treatment program. Hopefully this will help with my grief and guilt. As far as work goes I have been with this company for several years and they are absolutely great. They new of my conditions when they hired me for training under the wounded warrior program. They have done there best to facilitate my disabilities. After several full blown panic attacks and them seeing me in constant pain they allowed me to work from home. That was great for about a year but all the meds I am on has my memory shot. Then I started missing important clauses in contract reviews and writing incorrect information because I could not concentrate or focus. At this point they slowed down sending me stuff to do but carried me on payroll. Now one of the owners came by my house to talk to me and ask if I was willing to go on LTD. He told me he checked with the insurance and they told him I definitely qualify. He didn't have to do this but I did good work for them and made them a very large sum of money up to the point my disabilities overcame me. Now I no longer work for them but they still cover me and my family on there group health insurance until January. Since the VA has been providing me treatment for my PTSD and MH problems that is why I need them to fill out that portion. I don't see why they have not assisted because they are fully aware of my work problems and documented it. Even made statements to me and in my records I was not fit to work in stressful or environments that had potential to overwhelm me due to panic, anxiety and hallucinations that are associated with them for fear of harm to self or others. I have made copies of all these notes and diagnosis for the insurance company but without the paperwork signed by them I am screwed.
  4. The company I work for has given me paperwork from our insurance company for long term disability. My service connected disabilities are the reason I had to quit working. The company I worked for filled out there section, I filled out my section and now the physician section needs to be filled out and I am playing hell to get the VA doctors to fill it out. My primary care provider filled his section out without any problems but the section the psychiatrist and neurologist are to fill out needs done but I am getting nowhere with it. I was told they had to cooperate with the medical assessments. Is that true or can they decline to fill it out? It is very important these are filled out by them or the long term disability will be denied. Anyone know if there is a directive that tells them to assist
  5. I have fibromyalgia which I am rated 40% and after finishing a sleep study it was determined I have CFS. I plan on filing for CFS but have issues with Migraines. I was denied migraines before because they were only listed on my post separation physical. Since then I have tried everything under the sun. From acupuncture, Botox and many different medications. All of which failed. How is it that people are stating they can be related to Fibromyalgia? Is there any medical evidence that supports this or any BVA's that granted service connection as secondary to Fibro.
  6. I was planning on filling it secondary to fibromyalgia then the doctor doing the acupuncture listed the headache as occipital neuralgia. Screwed the whole thing up.

Berta, I feel it is from the constant strain and tension from my fibromyalgia but I do have 20% SC for a shoulder injury where I was thrown off my bike on my head and shoulder causing a 3rd degree AC cup separation. That happened in 96 or 97 but I have had headaches before that. I don't know maybe it did cause additional problems. I don't know?
  8. Berta, I am 90% overall and 40% fibromyalgia. I have been treated for years for migraines/headaches. Always assumed they were part of the fibromyalgia problems. I have even started gathering documentation on it. Until recently I started acupuncture for the headaches and after two treatments I read in my records I have occipital neuralgia. This was a first since from what I have read it is cause by a traumatic injury to the base of the neck or muscle damage or spasms. I was thinking that since the doctor providing acupuncture was also reducing the pain in my back in between my shoulder blades and my neck then that was a good connection between fibromyalgia and occipital neuralgia. Those areas are problems of my fibromyalgia but they are only under tension and pain. I haven't damaged them traumatically.
  9. Does anyone know if fibromyalgia can be a cause or associated with occipital neuralgia? I didn't know I had occipital neuralgia I just thought it was migraine headaches or tension headaches. I started receiving acupuncture and have some post stuck in my ears to block pain, anxiety, nightmares and so forth. Have been on multiple medications throughout the years with little success. Now I have ventured down the road with acupuncture and have been scheduled for Botox. I was planning on filing a claim for migraines/headaches related to fibromyalgia but now after reading the notes from my last two treatments I question the validity of my claim and if the two would be symptomatic of each other. Fibromyalgia and Occipital neuralgia. The headache is a headache either way you look at it but could or could not be related. Anyone have any experience in this area or know of the two to have a comparison that might substantiate the claim?
