Opioids, in general, suppress respiratory drive and have a depressive effect on the hypoglossal nerve that maintains airway patency. COPD is usually associated with an increase in arterial blood carbon dioxide levels. The administration of hydrocodone can increase CO2 levels, decrease blood oxygen, and result in an exacerbation of respiratory failure and CO2 narcosis.
