is this supposed to be a scan or just a pic or screen cap done on your phone?

If it is a true scan what device did you connect your phone too?

My iPhone is supposed to take a picture through the app “Notes”.  It scanned the entire document one time, but then I took it down to remove my ssn on one page.

Hey Okemos, I am hanging up on semantics. Scanning a document typically includes running the document through a physical device like a printer/scanner/fax machine and translating that into an electronic document. 

Taking a pic of a document is not typically the definition of scanning.

There is however new phone tech in alpha testing where a special camera does a "scan" using software based on QR code readers.

I was just wondering if you had somehow gotten access to that type of new phone.

I kept messing with it. The scan document attached to the very first email should be readable.

You're lucky you had Headaches separately rated. Usually, they're just combined with TBI. You can request an increase if they've gotten worse.
