It is true that the form is outdated but there are times when the VA will/would still accept the forms until a new form is created. Hope this works, the form did come up for me but not sure if I can attach it. The form says it has an Expiration Date: 09-30-2021. If you do it through va.gov directly it doesn’t matter what date the form is because you get whatever form is in the database and it has to be accepted. I emailed the form manager through the site about the link not working in the form look up.
