Examination For Housebound Status

Morning folks,

 

Just tried to download the VA Form 21-2680, Examination For Housebound Status.., and the VA Forms website said the form has been "Retired, no longer in use.."

Even the contact email for the Forms manager is no bueno.

Anyone know if the form has been replaced?

I checked internally this morning- the form posted here is the most recent available right now and when I open it from within the M-21 its the same form, form date, OMB Expiration, etc. M21-1 II.iii.1 A.1.a scroll through chart to the SMC/Housebound

Going to "walk" the Form to the Release Of Information folks at the VA hospital where my PCP is located.

I was told at one point to mail the 21-2680 directly to my PCP at the VA, but found out it doesn't work that way.

On another note, does my PCP at the VA have to process the form?

Some say yes, some say no.

No. They fill it out in the requisite sections but you have to send it in. Dont forget to date and sign.

Broken,

There is no place on the Form for the veteran to sign, just the examining doctor.

 

I filled in all the basics on me, name, address..etc.

Sorry about that. Its a habit- I get so many forms returned that I need to send back to the Veteran for a signature I just am in the habit of typing it for everything. Apologies.

I too type everything as I can't grasp and maneuver a pen very well.
