Anyway, I've got a veteran who's health is worsening, and his daughter would like to talk to his PCP and health team about a health plan for her dad. A member of the health team told the daughter that there are VA forms which allows her to discuss her dad's health with the doctors, nurses.., etc. Of course, no VA form number was mentioned, and the daughter left the VA hospital with nothing but a bad attitude! So, in view of the above, does anyone know what form(s) this poor young lady needs in order to discuss her dad's medical issues with the VA staff? Just a guess off the top of my head like everything else she would probably need a Medical Power of Attorney to discuss any health issues or treatment plans.
