Ftca Standard Form 95
Friday I went to a Federal Courthouse and asked for a copy of Federal Tort Claims Act Standard Form 95 and the clerk couldn't find that form in her computer or anywhere. Never heard of it. According to what I've read here on Hadit, that is the proper form for a Tort Claim. I thought I saw a site here on hadit once where you can copy that but I can't find it. Does anyone have that? Thanks, Mike

diabetes Got C And P Exam Results For Ibs
Any comments would be appreciated about this C&P exam, thanks: Opinion: Veteran does not have the typical pattern of Irratible Bowel. He deveoped diarrhea upon the initiation of Metformin (he could not make the connection, although he states he told his MD Diarrhea began when medication for diabestes was began and that his MD took a BM sample). He's had no work up for diarrhea, of course the simpliest way to know what's cause diarrhea is to stop the Metformin which is commonly complicated with diarrhea. Veterans diarrhea (not IBS) is less likely than not related to Diabetes Mellitus, as likely as not related to Metformin. ______________________________…
