All C & P Blank Worksheets

I have posted this here many times over the years and it still is VERY difficult to find:


Having the blank worksheet can give a vet a heads up on what to expect from a specific C & P exam.

Make sure to request a copy of the exam as soon as it is completed and written up.

I didnt know for many years that one could obtain a copy of a VA C & P exam.

I didn't find that out until I called up a VA doctor who did a C &P I questioned in 1998- I was furious with what the SOC said he said- he remembered the claim and had the actual C & P results in his desk.He started to read his actual C & P results to me over the phone-

wait a minute!-I said-

'the SOC was completlely different as to what you stated in the C & P.'

He sent me a copy right away.

I have used his actual C & P statement for my 2003 awarded claim, and for my present AO IHD claim as well it was significant to my former 1151 and FTCA awards.

Also I used it as "established' medical evidence for my 2 CUE claims pending under Nehmer.

The VA will skillfully manipulate the wording of a C & P exam result to our detriment -if they think they can get away with it.

They will also fail to give the C & P doctor sometimes, significant evidence they need.

This VA doctor was stunned to find out when I called him up-that he had made statements on the cause of my husband's death without even having a copy of the autopsy.

VA Buffalo RO even kept the autopsy from him and the VA Central Office Peer Review doctors.

Somehow they managed to lose 12 copies of it.

Every time I sent it to them with proof of mailing-it disappeared.

When I figured out what they were pulling-

I submitted it directly to the OGC and VACO-and then I won those claims right away.

It is the actual C & P results that matter-not the way VA might present the results in an SOC. Most of the time IO think they get it right in the SOC - but who knows how many go through what I did-

only to find out the SOC was all wrong.
