Question

Hi Everyone,

Its been some time since I've posted here and its been a long road. Currently my husband is Blind in one eye and losing his vision in the other, he is also on dialysis for Kidney Failure and in need of a Liver and Kidney transplant. My question is, Are there any benefits that I can apply for through the VA? He is also receiving ChampVA....

Thank you in advance:

Hey Philip,

What form would I need in order to file for Aid & Attendance for him?

I don't know if there is a specific form but you can just write a simple letter requesting A&A for your spouse. In addition to that letter, I would have his physician fill out VA form 21-2680, the A&A exam form and enclose that w/your letter requesting A&A. http://www.va.gov/vaforms/search_action.asp?FormNo=21-2680&tkey=&Action=Search

there is a VA FM 21-526b you can complete. 3/4 the way down the form there is a question if you are requesting benefits for a disabled spouse. Complete this form and attach all medical records that support his disabilities, to include the social security award.
