Should I have gotten a functional questionnaire form?

Question

I had a c&p on Sep 3. for back in jury exam. By reading some posts, many filled out a functional questionnaire before the exam however I never received one nor did I fill one out. The doctor was poor.  Should I ask my vso?

just because they did not fill out it out there doesnt mean it was not filled out, they can take their notes from the exam and fill it out later, hard to tell.  i would go and get a IME from a board certified specialist.  was your doc a Nurse practicioner or general practice? or were they a orthopedic type?

Yes, I believe it is a dbq. I never seen one. The doctor fills it out? or the patient?

In order for the VA to accept it as creditable a certified MD should fill it out and sign it.  You may fill one out yourself so that you are aware of what the doctor should be looking at.

To be honest bud, a lot of Vets including myself have been granted SC without a DBQ. They do help a lot, but most VA Docs wont or don't want to fill them out prior to your exam. As long as you had well documented evidence you should be fine. Sorry to hear the exam went bad, as we all know too well how that goes. Good luck and keep us posted.

I fell on my back while attempting to touch anti static stem on c141. 

Since 1990 have had bouts of muscle spasm and Djd. 

What are my chances of getting a rating? Everything documented and sent in.
