0 jfrei Posted December 17, 2017 Share Posted December 17, 2017 Actually I myself didn’t fill out a damn thing I called the champva phone number and as long as they are enrolled as dependants in the VA not sure if you said they had ID cards but you should be Able to do it over the phone speaking from experience just did this about 5 months ago Link to comment Share on other sites More sharing options...
