  1. Recieved 2 more forms today, both of which I already submitted but no one can find them. One is for the name of 3 people familar with my condition (health status) Second is actually a residual functional capacity form for me to fill out. Luckily I also have the one the doc signed to go with it. Odd these two papers were sent from Tampa but they get forwarded to Tenn. When I called Friday to ask where to send some other papers I was told to send them to the Tampa location. Well at least I am prepared to play thier game and wait.

  4. Trying to look at M21 and when I go to download or view it its says: can't load word for windows 6.0. I am truly computer illerate. I have Windows XP if that helps. Thanks for any help
  5. I tried calling SSA but didn't get through will try again Monday. I hope its the latter, I hate going to the VA to get a 3rd copy of my Xray's. The most importnat part of my claim is for a doc to review my x-ray's the alone should get me SSD.
  6. I submit everything under the sun to SSD to save time. I called DDS and they have no records of the records I gave SSA or the computer disk with copies of my Xray's on them. Looks like this is going to be a nightmare also.
  7. rentalguy, I think I have enough scarring to show they are frequent in nature and I have several that are present, right now I am on antibiotics, so they are not inflammed. Is bypassing the correct dx the best way to approach this. I have some BVA decisions that are favorable on this issue. What I have is a statement from the doctor saying hidradenitis is the probably cause of the ReA-now that I am reading his exact words. It doesn't say cyst, here is exact words She has radiographic sacroilitis bilaterally. Now she reports hidradenitis in groin for 30 yrs. Now she has axillary pustules as well. After reading this verbatim-he is saying I said I had HD-which isn't correct Next note says: 57 WF with reactive arthritis secondary to hidrandenitis ImP as above pt is competely permanently physically disabled. He has put this in his last 3 notes. In his last letter I wrote, he read and I did take my records for him to review says: Hidradenitis Supportiva is a know cause of RA and this condition more likely than not is contributing factor to her ReA. I have reviewed her smr dated-then the dates Again another statement completely physical permanently disabled. I guess I have no option but to go for a change in DX.
  8. I am SC for cyst 0% rated as a scar. Here's what a BVA wrote in 1986 New skin growths, benign, will be rated as scars. Superficial scars which are poorly nourished with repeated ulcerations or which are tender and painful on objective demostation will be rated at 10 percent. Other scars will be rated on limitation of function of the part affected. Here's the decision 1986 A cyst of the right thigh, not more than 2-3 mm in diamater, recurred in 1984 the cyst is not currently infected or otherwise shown to be symptomatic.--Increase was denied. My medical records both in service and va show that I was treated for an infected cyst/celluitis of the r thigh (groin) 4-5 times. Then I stopped going to the va and treated them myself. What I wanted to do was to bypass asking for a correct dx that will slow everything up--I wanted to ask for secondary due to cyst. Reactive Arthritis is caused by an infection. Simpliest dot connection here. I want to avoid getting denied due to 3.307--chronic within 1 yr ratable at 10%. If they had known what I had or I saw someone who knew while in the service or va then I would have gotten the 10%. If I go with the correct dx, then I will have to show that the first cyst was the beginning of the HD---since this apparently isn't a well know disease process, I don't want to get a examiner that is an idiot and I have to appeal.

  10. hurry and rental I don't know what I am asking for. I am asking I think for Reactive Arthritis (ReA) secondary to cyst. I am service connected for cyst o% (rated as scar) The cyst(s) are now dx as a disease and not just random cyst. "57 yo WF nurse with ReA since 1977" "Now she reports hydradinitis in groin for 30 yrs. Now has axillary pustules as well." IMP: reactive arthritis (suspected due to hidradenitis supportiva (spondyolarthopathy) I see problems, I didn't say I had hidradenitis, he said it. He said " I am sure in the past 30 years we ask you this, have you ever had any cyst in the groin or axillar area." I said yes and then showed him my axillar.

  12. Re: Reopen Claim I am requesting to reopen my claim for reactive arthritis (ReA) in all joints secondary to cyst (rated as scar 0% sc). As evidenced by the following it is my belief the ReA was the direct cause of an infected recurring cyst that developed while on active duty. In support of my claim I have included copies of pertinent service medical records and veteran medical records along with letters from those with knowledge of these frequent recurring cyst. 18 Dec 1975: 1 in tender node R med thigh hard. 19 Dec 1975: c/o of swollen lymph node in inner aspect of L thigh leg x yesterday-very painful- states she applied hot soaks to leg last PM with no relief I was leaving the base for leave on 20 Dec 1975, I was instructed to stop at any hospital should it become necessary for me to seek further medical attention for this infected cyst. The military facility refused to do anything further due to my leave. I was forced to stop and have the cyst lanced upon arriving in Tampa, I was to sick to drive to my home, I parked at a doctors office and waited for him. He lanced the cyst and instructed me to keep hot towels on it for it to continue to stay open and drain, he also started me on antibiotics. Within a few days later I developed a rash and I went to MacDill AFB, for medical attention. Undated note - This occurred within a week of December 20, 1975. MacDill Hospital. Infected sub cysts groin -no treatment ordered (saw private md couple of days before)-rash Scabies probably acquired in boot camp. 25 Jan 76 Staph infection-was treated by civilian doctor for staph infection (subcutaneous cyst in groin area without success pt c/o discomfort and fatigue. 26 Jan 1976: Staph and Scabies From January 1976 until Nov 1978, I used hot packs to treat any further cyst that occurred during this time. Nov 22 1978 at1035 am (2 mos post discharge from USMC) Fever 2-3 days large swollen area near perineum and top of L thigh: 22 Nov 1978 at 6:40 pm additional note due to allergic reaction- DX acute lymphadenitis infection Nov 29 78- Pt returns for follow up of cellulitis- resolution of celluitis (a cyst deleveoped and resolved during this time period). 5/21/84: Cyst medial infected R thigh. During a Comp Exam dated 5-1- 85 it was noted; cyst was still present 2-3mm very small. During all my encounters with the medical professionals regarding this matter, whether they realized it or not they always made me feel like I was dirty and unclean. I primarily didn’t seek medical attention for this problem, since they never did anything for them I applied hot soaks and attempted to get them to open. After I became a registered nurse with 20 years of critical care experience I felt comfortable lancing them using aseptic technique. As a registered nurse, I was able to obtain antibiotics from a physician if the need arose. There were frequent times that I was unable to accept work when I was working agency due to a severe flare up with numerous foul smelling cyst both in my groin and axillar. On xxx I informed my primary xxxx ARNP that I had been avoiding seeing the GYN physician due to frequent cyst and blackheads in my perinum/ groin area. On December 28, 2007, I was questioned by my rheumatoligst Dr.xxx, Chief of Rheumatology if I ever had any cyst in my groin area or under my arms. At that time I was having a severe flare up as noted in his notes. At that time Dr xxxx, Chief of Rheumatology reviewed portions of my service medical records and in his opinion the 0% SC for this cyst has been missed diagnosed and the correct diagnosis is hidradenitis suppurtiva and the reactive arthritis is secondary to this condition. On January 15, 2007, Dr. xxxx, Chief of Rheumatology again reviewed a portion of my medical records relating to these cyst and stated a second time "57 yr old WF with reactive arthritis secondary to hydradinitis suppurtiva. Enclosed are additional statements from close friends and family that are aware of the continuous problem I have had related to the frequent and recurring cyst. I hereby certify that th information I have given is true to thebest of my knowledge, memory and belief. Suggestion anyone, if not I sending it.

  14. Thanks Would you just start it off with I am requesting an increase for sc cysts. Then give all the dates you were treated for with copies of those records. Statement from MD that says cyst is HD and the ReA is secondary. Statements from others who knew you had this but took care of them yourself throughout the years. I have 2 signed letters from the Chief of Rheumatology stating he reviewed my records and its his opinion that more likely than not the ReA is secondary to the cyst (HD). He also states in these letters in his opinion I am permanently and totally disabled due to these conditions.
  15. Is there a way to check on a BVA decision from 86. I had a claim in 86 that was mostly denied for increase, I never followed through on it, and went my merry way away from the VA. I just found a letter dated 4/30/86 Your file has been temproarily transferred to the BVA in Washingtion, D.C. If you will want a hearing, we will recall your file from the BVA's which will result in delay of consideration by them. Plese advise this office immediately if you still wish a hearing. I am starting to understand now, I did get the BVA results and the back was remanded, and I didn't do anything. I didn't understand what I was suppose to do-long story. Does this mean if I take action now on this remand I get back pay from 1986 A remand means no action was taken, you need to do something to continue the claim is that correct. Can someone explain what happens to a case if it was remanded for further evidence before any action would be taken.

Reactive Arthritis- in all joints secondary to Cyst 3. Bilateral Carpal Tunnel secondary to Reactive Arthritis 4. Bilateral Heel entheospathy secondary to Reactive Arthritis 5. Bilateral Achillis tendonitis secondary to Reactive Arthritis

  19. I am having a problem also, I had the hadit site on favorite and now I get can't find that site. I put it in manual and got a new home page, when I hit on forums it takes me to hadit forms.
  20. Is this correct? She placed the restriction a month before the exam? The exam was held April 12, 2005 and the records show she placed on March 16, 2005: If this is true I would think this note preceding an exam, would show some pre-conceived prejudice against the patient. The exam that took place was done in such a way to justify that prejudice. If they claim error, then there is an issue of competency, more than one note or notes with erroroneous dates, one being an implied exam or fraudulent exam notes from a CE that never took place. Notes made prior to an exam. While this may be a practice that is kosher for the VA, I don't believe it would be kosher in civilian law. If worded correctly and sent to the right people could cause this psy facility some problems. While the outcome of any inquiry you do may not be what you might want, it will possibly change the way things are done. Should you lose this, you may have other options as a result of these actions. Have you every gotten a copy of your C file to see what they RO ask them to do at the April 2005 exam and what or why there is a notation for exam in 2007. The RO/DRO (whomever) may have asked for them to review thier records again. I am aware those notes are now missing---Which is illegal even for the VA.
  21. Yes, I am understanding it better. I will write more tomorrow,its late, you have some issues to address to someone. Take your time and start writing what happen as clearly as you can step by step. Remember the person reading this knows nothing, while you should try to keep it to the point, don't leave anything out. Dr. L and Dr. Bxxx were both in the room. She did not do any talking only he did. This would be Dr. Bxxxx. Dr. Bxxxx only acknowledged the C&P. Dr B ask all the questions, but Dr L is the one that wrote the exam up and Dr B cosigned the note, your complaint was against Dr L, is this correct? Did you file the complaint prior to getting a copy and reading it or right after the exam? 30 Days after the compliant was filed she blocked your access to your file or just that note of April 2005? Trying to figure out if you had read the CE- then made a complaint or filed the complaint and then got a copy of the CE There was no exam in 2007 just a note saying you were examined.
  22. Anything I say is based on civilian knowledge and some VA experience-I worked as contract nurse at the VA, I know some of the rules and how they react to inquiry's. They shake in thier boots if you do it right. The VA system is very political. When it comes time for promotions at any level they don't want any black marks. I haven't followed all your threads regarding this matter so,please forgive me and tell me the following. You had a CE in 05 as a result of this CE you filed a NOD (or whatever) What did you complain about and when (original complaint) Who did you see, is the person that did the exam one and the same or did you see someone else and this Doc co signed the notes. If the amendment is now gone, go to the VA and ask for that specific date if you can. Don't know about psy notes. Was your complaint, that she made a statement you could not view this note? AUTHOR: LEHMXXXXX LXXXX EXP COSIGNOR What does EXP COSIGNOR mean? What did she exam? Your notes? Did she imply she examined you one on one? Are they required to exam the person or just the file? Those of some of the questions you need answers too. Before you file any complaints to anyone, you want to make it look professional and your concerns are real and that you are not some wacko person trying to discredit someone you disagree with. If they did make an entry that implied they interviewed you one on one and you can get those notes, you gottem. If you can show that those notes disappeared--its enough to cause someone a really big problem. If they removed the addendum saying you couldn't view after review of your complaint they simply did as you ask.
