PCP Writing Bad Notes in Medical Records

Then ask her in a nice way to amend these statements...I don't think they can take them out of the Notes but they can amend them.

its better to try to get this solve with your PCP other than just get a new PCP  solve this issue first and then request another PCP.

They will want to know why...this is your chance to let them know how well the healthcare is at your VAMC.

I did this with one of PCP RN 's

That's genius. I already know that if I asked my PCP that question, they probably would be totally clueless...

The crazy one here, the next time you go to one of your PCP appointments ask her for a consult or a referral to PT (Physical Therapy) and or Nuero Surgery to take a look at your back condition, be insistent but be polite, get a medical opinion. You can ask your PCP for any and all referrals that you feel that you need.  IMHO, don't worry about the bad comment just get medical evidence to prove your conditions.  Many years ago, I had a doctor write a similar negative comment in my records and it caused me some problems with my claims until I had a few specialist review my medical records and give their opinion and everything changed.  It was like dominoes after that, my claims went through without much of a fuss.

And! Use MHV Secure Message to give a detailed error report to the Dept or Clinician involved. A copy of your MSG will appear in your MHV Med Records. When you receive the Secure Msg response from your Clinician, informing you of the VA Official Form to add your opposition statement, make a copy of that also.

Secure Msg's remain on you MHV site for about 12 months. The eronius Clinician "Progress Note" will not be removed from your VMC medical record, however your Secure Msg and Official Report of Error will be always become part of the file.

I've personally had to do this on (2) occasions, PCP intake nurse checked off questions we never discussed. Both occasions, I notified Dr with Secure Msg and the message was in my MHV Records within a week. As far as I was concerned, that was enough to put what the Nurse's notes into question. I opted not to file the offical VA Form. The nurse has not made the same mistake since.

I'm having these same issues as well and I've used MHV to contact my PCP thru whoever it is that plays the intermediary. They were also forwarded to the Pat. Ad. at the same time and I've never had such a quick response from my PCP ever before.

I'm so sick of it you can't imagine. From now on I review those records when they become available and if there is anything and I mean anything in there that we didn't talk about and doesn't smell of an opinion, they're hearing from me and the word "complaint" is in the vocabulary when I contact them. I don't give a rats behind if they call me a complainer or whiner, I'll just call them a liar!

I use secure message to draw attention to VA mistakes and record problems I am having with medical issues without going to VA. It works.
