Ok I feel I really have a case for 4 yr the director of the vr&e has held medical devices from me that were grant.

They were aware of the problem I was have cooking feeding myself I have lost over 15lbs I take vitiam b12 shots my health has gone down have medical proof of this.

Progarm is to keep track of your health and make report never don

The program can last 24 months it been 4 yrs what is they puting in computer.

I have email with the regional drictor about this.

4 yr ilv I got a box of railing from hospital and ilv never would put them into my house.

I moved out my own house because I could handle thing ilv was aware not one referral any were.

Never referred for a&a now va hostipal is paying for 8 hrs of in home care.

My ? Is I wrote it and send it with damage amount. Do I still need to fill out there form.

NEED MORE INFORMATION ON YOUR MEDICAL CONDITION  AND IS IT SERVICE CONNECTED? 

Are you in Appeals status?

Are you asking how to get medical devises from the VR&E Program/ the counselors are holding up for the things you need around the house?

Are you 100%S.C. Now? 

Are you in the VA  M.O.V.E. Program? (you lost 15lbs)

Are you wanting to get SMC? =   SMC H.B  or A&A ? 

Are you wanting an Increase in your present S.C. Condition?

Are you wanting to know about how they have you rated at present?

or

If your wanting to file a claim because your S.C. Disability has worsen and you can't fend for yourself  (feed your self, dress your self  & need help with mother nature ect,,,ect,,,,...

You need to file for increase and submit all your medical records and any Dr.s Notes Describing your Health Condition, you can ask for A&A (Aide and Attendance) or let the VA Decide a Referral to SMC based on the medical evidence you send in.

Remember probative /credible  Medical Evidence on your S.C. condition is what helps win these claims.

 Qualified Dr's favorable Opinion, Specialist Notes in your favor,

You are very hard to understand. I don't know what "ilv" is or what is damaged. What form are talking about? Take your time and try to be a little more specific about your issue.

what was the FTCA claim for?

Current disability, rating, and status (housebound etc...)

transportation means?

I think you understand exactly what you mean, but a lot of the ppl here miss it, if you fill in the blanks, it will help get someone to help you, ok? I think your  backstory is more complex than anyone here knows, so start at the beginning please.

This is the administrative claim I put in before I file a tort suit.

I was accept in ilv program 4 yrs ago. Vr&e has just held my case never did any thing

I got divorced an apply for service. The director has just leftmy case open with no report. No referral for help for me.

Now I have to take b12 shot daily. The hospital is paying for pcs care in my home now.

This just some of the evidence I am using for my tort claim.

I don't understand what your asking?

I assume you have Pernicious Anemia  Taking the B-12 Injections.

I have to take the B-12 injections twice monthly at 1000 MG that not to bad  but it keeps my tingling and swelling  under control  no compensation for this Unfortunately..

and I think I got the Pernicious Anemia from Vietnam for what ever reasons they could be   its just not on the AO. Listings 

As for as  ilv   Not sure what that means? or do you mean the ILP (Independent Living Program?)

VR/E  Should not hold any claims? not sure what you mean by that?

Have you tried calling the 1-800# and ask them the status of your claim  or tort claim?

Maybe some other Hadit members can chime in..I don't know what to say to you?
