Sf 95 - Johnbeckham87
Berta Can you give some advice if I have a SF 95 claim. I was found to have spina bifida in July but the va diagnosed a back sprain to fit the 10% they gave me in august. I was not aware of the spina bifida but complained of symptoms ( uti's, loss of feeling in legs and neck pain which caused a secondary disability . The non treatment of the symptoms affected my work performance and caused me to miss work which caused me to be fired. Since the va was aware of this but did not treat me because they assumed it was a back sprain would this miss diagnosis be considered mal practice?

ptsd Tort Claim For Slip And Fall On Va Sidewalk
In august, after leaving the DC VA emergency room (high on pain killers), my husband was pushing me in my walker (extreme sciatica pain - which is why I was at the ER, plus I was under the effects of a pain shot), my walker's wheel got caught on a large crack on the sidewalk and I fell backwards onto the cement. I am an MST/PTSD vet, I had a panic attack and refused to return to the ER. Also, since I had received a shot for pain, I was not feeling any. I did return to the ER two days later, because when I woke up the next day, I could not walk at all. The pain shot had worn off and I was in extreme pain. I am filing a tort claim. I already am rated for her…

ptsd Va Dr's Started Operating On Wrong Vertebrae
I underwent an L5-S1 TLIF surgery on 1-19-2013 at the VA hospital in Houston. When I woke up my wife informed me that the Dr's told her that everything went well, except for the fact they started on L4. They told her that they had cut bone out of the way and also drilled holes before they realized they were at the wrong level. They stated that when they realized it they replaced all the bone fragments to allow them to re-fuse (hopefully). The next morning when the Dr made his rounds he told me everything went well, my son was with me in the room. I asked him about the mix up and he said that yes, they had cut bone from the wrong vertebrae and that it would certainly cau…

Should I File An Sf95?
In Nov 2011 went to VA ER department for bronchitis, while there they done a chest x-ray. The ER doctor informed me I had a nodule in my lung in the left upper lobe. I was informed it was on the x-rays done back in Feb and April of 2010. He could not believe I was not informed of this nodule, as they were supposed to put in a consult for it back in Feb and April of 2010. He ordered a CT Scan with contrast. for concern of cancer. A week later they did a bronch wash, the doctor performing this told me as he walked out the door that it was not cancerous and that I had a fungul infection. I was later told it could take up to 12 weeks to get all the results back. I als…
