Shreddergate and other scandals
On the 13th of October, 2008, VA Watchdog dot Org broke the story about documents found in shredder bins at the VA's Detroit Regional Office.
A number of those documents were critical to veterans' disability claims.
As the story unfolded, VA finally began to release information on the incident and we learned that more than one Regional Office was involved in this shredder scandal.
The VA's Office of Inspector General (VAOIG) is investigating.
VA has instituted new guidelines regarding handling of documents.

Hospital staff said starting December 1st, the emergency room will only be open from 8:00 a.m. to 6:00 p.m. seven days a week. "SACRAMENTO - The Veterans Administration confirmed that a "do not resuscitate" armband was mistakenly placed on a patient who died following surgery Friday at the Sacramento VA Medical Center, but insisted the error was not fatal. "A U.S. District Court judge in Little Rock recently found that a surgeon at the Department of Veterans Affairs hospital in Little Rock had botched a surgery and awarded the 75-year-old patient $725,000. In a strange twist of fate, as my right arm is in a splint temp cast, I've been looking through my records to pass time as I can't fly my RC s while in this temp cast. Although I won't get into the specifics of what was omitted, I have come across two documents which have been altered and are missing the physician's notes. I have been diagnosed with anxiety and depression since 2010.
